Qualifications

• Minimum 1 year of physical therapy experience, preferably in home healthcare.
• CPR certification required.
• Knowledge of home health regulations is essential.

Responsibilities

• Providing home-based physical therapy to patients with mobility, strength, and functionality issues.
• Collaborating with a home health team including RN, LPN, CNA, HHA, OT, and SLP.
• Assessing, documenting, and modifying care plans as needed.

Key responsibilities of a Home Health Physical Therapist include:

• Providing home-based physical therapy to patients with mobility, strength, and functionality issues
• Collaborating with a home health team including RN, LPN, CNA, HHA, OT, and SLP
• Assessing, documenting, and modifying care plans as needed
• Assisting with exercises and hands-on therapy to manage pain and improve mobility
• Educating patients and families about care plans and exercises

Must-have qualifications:

• Graduate of an accredited Physical Therapist Program and active PT license in IL
• Minimum 1 year of physical therapy experience, preferably in home healthcare
• CPR certification
• Knowledge of home health regulations
• Strong clinical judgment, interpersonal skills, dexterity, and compassion
