Qualifications

Overview

We are seeking dedicated Physical Therapists to provide personalized, home-based therapy to patients in Lake & McHenry Counties. Positions available include PRN, Part-Time, and Full-Time roles. This role involves helping patients regain strength, manage pain, and achieve their goals within their home environment.

Key Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury
• Collaborate with a multidisciplinary team including RNs, LPNs, CNAs, HHAs, OTs, and SLPs
• Assess, document, and modify care plans based on patient progress
• Assist with exercises and hands-on therapy to improve mobility and reduce pain
• Educate patients and families on care plans and self-care techniques

Minimum Requirements

• Graduate of an accredited Physical Therapist Program and active PT license in IL
• At least 1 year of experience, preferably in home healthcare
• CPR certification
• Knowledge of home health regulations
• Strong clinical judgment, interpersonal skills, and compassion
