About the Role

Empath Home Health, a member of Empath Health, is seeking a highly motivated and compassionate Home Health Physical Therapist (PT). The Physical Therapist treats patients using mechanical means such as exercise and heat to help them restore and maintain optimal physical functioning.

Locations

Home health visits in North Sarasota, Longboat Key, University Parkway, Downtown Sarasota, Fruitville, & occasionally Siesta Key.

Schedule

Full-Time, Monday to Friday, with weekend on-call and holiday rotations.

Responsibilities

1. Provide physical therapy services as prescribed by a physician, adhering to agency policies, procedures, and regulations.
2. Perform initial and ongoing assessments, including OASIS assessments, to determine patient’s level of functioning and ability to perform daily activities.
3. Develop and revise care plans in collaboration with physicians and care team members to help patients achieve mobility goals.
4. Report patient responses to treatment and any condition changes to physicians and agency representatives.

Requirements

• Licensure as a Physical Therapist (PT) in Florida.
• At least one year of experience in home health care is preferred.
