Job summary

Seaport Scripps Home Health seeks a licensed Physical Therapist to provide in-home therapy across the South Coastal Territory in the San Diego area. The PT will implement personalized treatment plans in patients' homes to help regain mobility and independence.

Responsibilities include evaluating function, educating patients and caregivers, coordinating with physicians, supervising assistants, and documenting progress.

Qualifications

• Active California Physical Therapy License.
• Minimum 2 years of physical therapy experience (home health preferred).
• Strong communication and organizational skills.
• Current CPR certification.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Evaluate patients’ functional status and develop care plans.
• Assess home environments and recommend modifications.
• Provide direct physical therapy treatments.
• Educate patients and caregivers on therapeutic techniques and equipment.
• Monitor progress and adjust treatment plans.
• Collaborate with physicians and interdisciplinary teams.
• Document clinical notes and progress reports.
• Participate in discharge planning.
• Supervise Physical Therapy Assistants per policy and regulations.
