Responsibilities

• Provide direct patient care and evaluate functional needs and outcomes
• Design and implement individualized care plans
• Coordinate with support staff to meet patient goals
• Document evaluations, treatment plans, and progress
• Lead and support team members and promote community physical therapy
• Work in a physically demanding environment, including patient handling and transfers
• Potentially work irregular hours, including call hours

Minimum Requirements

• Graduation from an APTA-accredited physical therapy program
• Current, unrestricted PT license for the state of practice
• At least one year of healthcare experience
• Willingness and ability to travel within the coverage area
• Reliable vehicle, valid driver’s license, and auto insurance
• Ability to lift 50-100 lbs and perform physical tasks

Reporting to the Branch Director or Supervisor, you will be a vital part of our healthcare team.
