Physical Therapist (PT) - Home Health - Far SouthWest Suburbs

Join Advocate Health as a Physical Therapist (PT) - Home Health serving the Far SouthWest Suburbs, including Midlothian, Crestwood, Robbins, Blue Island, Calumet Park, and Alsip.

Location: Far SouthWest Suburbs with focus on specified areas
Hours: Days, with rotating weekends and one summer/winter holiday
Sign-on Bonus: Up to $10,000 (must be employed in the same role at payout; internal team members not eligible)

Responsibilities include:
  • Ensuring and improving quality of care by utilizing appropriate resources, adhering to evidence-based standards, and promoting patient safety.
  • Participating in care coordination, team meetings, and organizational initiatives to support comprehensive patient care.
  • Demonstrating leadership and professional development through sound clinical decisions, effective communication, and interdisciplinary collaboration.
  • Developing and maintaining restorative patient relationships, exceeding expectations, and promoting self-care and disease management.
  • Engaging in business development, resource utilization, and patient education to enhance service delivery and patient outcomes.
Licensure & Qualifications:
  • Licensed or eligible for licensure as a Physical Therapist in Illinois.
  • Minimum one year of clinical experience or strong clinical affiliation experience.
Skills & Abilities:
  • Proficiency with computer systems, strong documentation, interpersonal, and time management skills.
  • Ability to use home therapy equipment and assistive devices effectively.
Physical & Working Conditions:
  • Extensive travel within the service area; must have reliable transportation.
  • Ability to lift up to 35 pounds and perform patient transfers safely.
  • Work environment includes patient homes and assisted living facilities.
