We are seeking compassionate and dedicated Physical Therapists (PT) to join our team in Central New York. This position involves servicing adult and geriatric clients on a per visit basis throughout Herkimer County.

Requirements include at least one year of clinical experience as a licensed Physical Therapist. As a home care PT, you will be an essential part of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities

1. Conduct home visits to clients in designated areas.
2. Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Create and implement individualized care plans including therapeutic exercises and training.
4. Assess and update care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative care to promote health and independence.
6. Document all observations and interventions accurately using electronic records.

Qualifications

• Current NY Physical Therapy (PT) license.
• Minimum of one year recent experience as a PT.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency in computer skills; experience with EMR preferred.
• Ability to travel within assigned areas.

Additional Information

Base pay ranges from $60.00 to $64.00 per point, depending on qualifications and negotiations.
