We are recruiting a compassionate Home Health Physical Therapist (PT) to serve patients in Southwest Missouri. Positions are available in Springfield, Monett, and Joplin. These are full-time roles.

What you'll do:

Visit clients in their homes to assist recovery from injury, illness, surgery, or hospitalization.

Qualifications:

• Active and current PT license
• Prior home health experience is welcomed but not required; training provided
• Ability to travel locally to cases

Responsibilities:

• Make home visits in designated areas
• Conduct diagnostic tests and assessments
• Implement wound care interventions
• Develop and revise individualized care plans
• Educate clients and families
• Document care activities accurately using a touch pad tablet
