PT-Home Care Physical Therapist-Per Visit

Position Overview: As a Fee-For-Service Physical Therapist at Your Choice At Home, an affiliate of Cobble Hill Life Center, you will evaluate clients' physical functioning at home and collaborate with clients, families, and healthcare professionals to plan, provide, and coordinate physical therapy services according to MD prescriptions. You will work with multidisciplinary teams to establish rehabilitation and maintenance goals.

Responsibilities include:
  1. Developing treatment plans that include modalities, visit frequency, expected outcomes, and goals.
  2. Implementing treatment plans under physician guidance and in collaboration with agency staff.
  3. Monitoring patient reactions, documenting progress, and reporting changes to physicians and team members.
Qualifications:
  • Valid NY State Physical Therapist license and registration.
  • Graduation from an APTA-approved physical therapy program.
  • At least 1 year of experience in hospital or long-term care settings.
  • Experience with pediatric populations required; experience with diverse age groups is a plus.
  • Knowledge of Medicare/Medicaid and credentialing standards.
  • Reliable transportation and comfort with home visits in various environments.

Physical demands include standing, walking, bending, reaching, climbing stairs, and lifting up to 50 pounds unassisted, with accommodations as needed.
