Job summary

Compassus is seeking a Home Health Physical Therapist to deliver patient-centered care in the home setting. You will assess, treat, and help patients regain independence, enabling them to live safely at home and participate in daily activities.

Your role emphasizes one-on-one therapy, collaboration with care teams, and delivering compassionate, evidence-based interventions with flexible scheduling and a supportive culture.

Qualifications

• Minimum qualifications include a Bachelor of Science in Physical Therapy.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• CPR certificate, negative TB screen and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting.
• Good interpersonal skills and knowledge of durable medical equipment.

Responsibilities

• Provide one-on-one home health physical therapy to patients in their homes.
• Assess, treat, and help patients regain independence and participate in daily living activities.
• Develop and adjust individualized treatment plans and goals.
• Educate patients and families on exercises, safety, and home exercise programs.
• Collaborate with the care team to optimize patient outcomes.
