Job summary

Compassus is seeking a Home Health Physical Therapist to assess and treat patients in their homes, helping them regain strength and independence. You will work one-on-one with patients, develop personalized plans, and collaborate with a care team to ensure safe, effective outcomes.

Requirements include a Bachelor of Science in Physical Therapy, current state license, driver’s license, CPR/TB/Hepatitis clearances, and at least two years of PT experience in acute or rehab settings.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• CPR, TB screen and Hepatitis consent/declination current.
• Two years’ experience as a PT in acute care or rehabilitation setting.

Responsibilities

• Provide home-based PT assessments and treatments to patients in their homes.
• Develop and implement individualized care plans and goals.
• Educate patients and caregivers on home exercises and safe activity.
• Document progress and coordinate with the care team to adjust plans as needed.
