Job summary

GrandCare Health Services is seeking a licensed Physical Therapist (PT) to provide in-home therapy across Long Beach, Cypress, Lakewood, and surrounding communities. You will evaluate patients, develop and implement care plans, and collaborate with physicians to support recovery at home.

Qualifications

• Current California Physical Therapy License.
• 2+ years of physical therapy experience.
• CPR certification.
• Valid driver's license with insured vehicle.

Responsibilities

• Perform home health physical therapy evaluations and develop care plans.
• Provide therapeutic treatment and instruct patients/caregivers.
• Monitor progress and report changes to physicians.
• Participate in discharge planning and interdisciplinary case conferences.
• Supervise Physical Therapy Assistants per state regulations.
