Job summary

GrandCare Health Services, part of The Pennant Group, is hiring a Physical Therapist (PT) to provide in-home therapy across Southern California, including Long Beach, Cypress, Lakewood, Cerritos, and surrounding areas. You’ll evaluate patients, plan care, and guide recovery in the comfort of their homes.

This role offers autonomy and flexible scheduling with per-visit compensation, bonuses, and reimbursement programs. CA licensure, CPR, and a valid driver’s license are required.

Qualifications

• Active California Physical Therapy License.
• At least 2 years of physical therapy experience.
• CPR certification required.
• Valid driver's license and insured vehicle.

Responsibilities

• Perform home health physical therapy evaluations and develop care plans.
• Provide therapeutic treatment and instruct patients/caregivers.
• Monitor progress and report changes to physicians.
• Participate in discharge planning and interdisciplinary case conferences.
• Supervise Physical Therapy Assistants per state regulations.

Pay: $80,000-$155,000 (pay per visit model)

Compensation Package
