TheraCare is seeking a licensed Physical Therapist (PT) for a PRN home health opportunity in Sherman, Denison, Whitesboro, and Gainesville, TX. You will provide in-home physical therapy to adults and seniors, performing evaluations, developing plans of care, and delivering evidence-based interventions in patients' homes.

The role offers flexible scheduling, autonomy, and the chance to make meaningful improvements in mobility and independence while collaborating with an interdisciplinary team.

Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active PT license in Texas or eligible for Compact Licensure.
• Home health experience (1 year) preferred.
• Knowledge of OASIS and home health regulations preferred.

Responsibilities

• Perform home health PT evaluations and re-evaluations.
• Develop and update individualized plans of care (POC).
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical, and complex conditions.
• Deliver interventions for mobility, gait, balance, fall prevention, and functional independence.
• Educate patients and caregivers on home exercise programs and safety.
