TheraCare is seeking a licensed Physical Therapist Assistant (PTA) for a PRN home health position serving San Antonio and surrounding North Central areas such as Canyon Lake, Bulverde, and Spring Branch. You will work one-on-one with adult and geriatric patients in their homes, implementing POC plans and promoting mobility, safety, and independence.

The role offers flexible scheduling, autonomy, and opportunities to contribute to patient recovery and quality of life through in-home PT services.

Qualifikationen

• Active Physical Therapist Assistant (PTA) license in Texas
• 1 year home health experience
• Strong communication and time-management skills
• Current CPR certification

Aufgaben

• Provide skilled home health PTA interventions under supervision of licensed PT
• Implement individualized Plans of Care (POC) from supervising PT
• Treat orthopedic, neurological, cardiac, and post-surgical conditions
• Educate patients/caregivers on HEP and safety
• Monitor patient progress and document in compliant EHR
• Adhere to infection control and home health safety protocols
• Support QAPI and performance improvement initiatives
