Job summary

TheraCare Home Health in Houston area is seeking a licensed Physical Therapist (PT) for full-time, in-home care across Baytown and nearby TX communities. You will perform evaluations, develop plans of care, and deliver evidence-based therapy in the patient’s home.

The role offers flexible scheduling, clinical autonomy, and one-on-one patient care, with opportunities to supervise PTAs and aides while coordinating with the care team.

Qualifications

• DPT from an APTA-accredited program.
• Active PT license in Texas or eligible Compact Licensure.
• Home health experience (1 year) required.
• HCHB experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Knowledge of OASIS and home health regulations preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop, implement, and update Plans of Care based on patient goals, diagnoses, and orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical, and complex cases.
• Deliver interventions focused on mobility, gait, balance, strengthening, fall prevention, and independence.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and modify treatment plans as indicated.
• Complete documentation within the EMR at the point of care.
• Coordinate with physicians, nurses, case managers, PTAs, and aides.
• Supervise PTAs and home health aides in line with state regulations.
• Participate in case conferences, discharge planning, and care coordination.
• Ensure Medicare/compliance and infection control adherence.
