Job summary

Interim HealthCare of West Palm Beach is seeking a PRN Home Health Physical Therapist to provide one-on-one therapy in patients’ homes across Delray Beach, FL. You will develop individualized treatment plans that restore mobility, independence, and quality of life, while delivering compassionate, patient-centered care.

This flexible role offers weekend visits, competitive pay per visit rates (63/93/115 depending on visit type), and opportunities for online training and CEUs.

Qualifications

• Graduate of an accredited Physical Therapist Program.
• Active FL PT license.
• CPR certification.
• Knowledge of Home Health regulations.

Responsibilities

• Provide home-based physical therapy to patients with mobility limitations.
• Work as part of an interdisciplinary home health team.
• Observe movements, conduct assessments, document progress, and modify the care plan.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on plan of care, goals, and self-care.
• Coach family members on safety and therapy goals.

As a Home Health Physical Therapist, here’s what to expect:

• Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises, and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few requirements for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in FL
• Minimum of 1 year of physical therapy experience in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgment, strong interpersonal skills, dexterity, and compassion
