Position Overview

Banner Home Health is seeking a dedicated Physical Therapist to provide exceptional care in patients' homes within the East Valley (Pinal County; Coolidge and Florence). This full-time, day-shift position offers autonomy, support, and opportunities to work with diverse clinical conditions.

Responsibilities

1. Evaluate patient abilities, develop and implement individualized treatment plans, and adjust as needed.
2. Document patient care according to regulatory standards, ensuring clear and timely records.
3. Communicate effectively with patients, families, and interdisciplinary teams to optimize outcomes.
4. Supervise and delegate tasks to support staff, ensuring safe and effective care.
5. Participate in departmental meetings and quality improvement initiatives.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current Physical Therapy license in Arizona.
• BLS certification.
• Ability to become proficient with electronic health records and clinical tools.
