Position Overview

Banner Home Health is seeking a dedicated Physical Therapist to provide care in the East Valley (Arizona City, Eloy, Casa Grande, Maricopa, Chandler). This full-time position offers a $15K Sign-on Bonus and relocation assistance. Work hours are scheduled Monday - Friday, 8 am - 5 pm, with visits in the patient's home to foster independence and recovery.

Responsibilities

1. Evaluate patient abilities, establish care plans, and set measurable, patient-centered goals.
2. Implement personalized treatment plans considering patients' physical and socio-cultural needs.
3. Document care according to regulatory standards, ensuring clarity and timeliness.
4. Communicate effectively with patients, families, and healthcare team members to coordinate care.
5. Supervise and delegate tasks to support staff, ensuring competency and safety.
6. Participate in departmental meetings and quality improvement initiatives.
7. Engage with multidisciplinary teams and external contacts to support patient care.

Minimum Qualifications

• Graduation from an APTA-accredited school of Physical Therapy or equivalent.
• Current Physical Therapy license in Arizona.
• BLS certification.
• Ability to become proficient with electronic medical records and patient care equipment.

Preferred Qualifications

Additional related education or experience is preferred.
