Job summary

Amedisys is seeking a capable Physical Therapist to support patients in home health settings. You will assess, treat, and re-evaluate patients, focusing on movement, activity, and functional goals, while coordinating with care teams and discharge planning.

Qualified applicants hold an active PT license in the assigned state and CAPTE-accredited program graduation, with CPR certification and relevant home health experience preferred. Compensation reflects market variations and location.

• Graduate from a CAPTE-accredited physical therapy program.
• One year of related physical therapy experience for home health care.
• If less than one year of experience, mentoring/approval from regional clinical leaders is required.

• Provide interventions, conduct reexaminations every 30 days, modify interventions as necessary, and perform discharge planning.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
