Job summary

Empath Health is seeking a compassionate full-time Home Health PT to provide individualized care across Hendry County, including Clewiston and surrounding areas. You will perform initial and ongoing assessments, design and adjust care plans, and collaborate with physicians and the care team to promote mobility and independence.

The role requires active Florida PT licensure, at least 1 year of PT experience, and a preference for home health experience.

Qualifications

• Active Florida PT license required.
• Minimum of 1 year experience as a licensed Physical Therapist.
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Provide physical therapy services to patients as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, to enable patients to achieve realistic mobility goals.
• Report to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition.

Empath Home Health, a part of Empath Health is seeking a compassionate full-time Home Health PT Physical Therapist to work with us to provide individualized care in Hendry county (Clewiston, LaBelle, Lehigh Acres, and surrounding areas).

Locations:

Home health visits in Hendry county (including Clewiston, LaBelle, Lehigh Acres, Moore Haven, Buckingham, and surrounding areas)

Schedule:

Full-Time; M-F 40 hours with required weekend/holiday rotations as needed

What you'll Do

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.

What You’ll Need
