Job summary

Elara Caring is seeking a dedicated Physical Therapist to deliver in-home physical therapy in the Burleson, TX area. You will assess patients, design individualized plans, and provide hands-on therapy in the home setting.

You will collaborate with physicians and caregivers, document progress, and supervise therapy assistants as needed. APTA-approved education and an unrestricted PT license are required, along with a valid driver's license.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid drivers license or have reliable transportation to travel between patient homes and assigned locations.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
