Concept Rehab, Inc. in Traverse City, Michigan is seeking a Physical Therapist (PT) to join our post-acute rehabilitation team. This full-time position offers mentorship, a stable schedule, and opportunities to impact adults and geriatric patients.

You will evaluate and develop treatment plans, collaborate with a interdisciplinary team, ensure Medicare-compliant documentation, and help patients regain mobility and independence. New graduates are encouraged to apply.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license or eligibility.
• Strong communication and clinical skills.
• Experience in Skilled Nursing, Post-Acute Care, Inpatient Rehab, Home Health, or Outpatient is preferred but not required.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate and develop individualized treatment plans
• Provide evidence-based physical therapy services to adult and geriatric patients
• Help patients regain strength, mobility, balance, and independence
• Collaborate with PTAs, OTs, OTAs, SLPs, nursing, and physicians
• Complete timely documentation in accordance with Medicare and regulatory guidelines
• Participate in interdisciplinary care planning and discharge planning
• Deliver outstanding patient-centered care while achieving positive clinical outcomes
