Job summary

Continuum Therapy Partners, LLC is seeking a licensed Physical Therapist for a full-time role split between Canterbury Place in Pittsburgh and Seneca Place in Verona, PA. The schedule is 3 days at Canterbury Place and 2 days at Seneca Place, providing PT services to geriatric and post-acute residents.

You will collaborate with an interdisciplinary rehab team to improve residents’ mobility, strength, balance, and independence in a skilled nursing setting.

Qualifications

• Graduate of an accredited Physical Therapist program.
• Current Pennsylvania PT license or in process of getting one - NEW GRADS WELCOME!
• CPR certification preferred.
• Skilled nursing experience preferred but not required.
• New graduates encouraged to apply.

Responsibilities

• Provide physical therapy treatments under the direction of the Physical Therapist.
• Document patient progress.
• Collaborate with nursing and interdisciplinary teams.
• Assist patients in improving mobility, strength, and independence.
• Follow physician orders and facility policies.

Continuum Therapy Partners is seeking a licensed Physical Therapist (PT) for a full‑time position that will be split between two skilled nursing facilities, Canterbury Place in Pittsburgh and Seneca Place in Verona, Pennsylvania. Schedule will be 3 days a week at Canterbury Place and 2 days a week at Seneca Place.

Join our experienced rehabilitation team providing high‑quality physical therapy services to residents in a collaborative SNF setting. Work alongside Physical Therapists, Occupational Therapists, and Speech‑Language Pathologists to provide exceptional rehabilitation services in a skilled nursing setting. Treat primarily geriatric and post‑acute patients while helping them improve mobility, strength, balance, and independence.
