Job summary

Outcome HC is seeking a compassionate Physical Therapist to join our Skilled Nursing Facility team at Mahoning Post Acute Nursing and Rehab. This full-time role focuses on helping older adults regain function, maintain mobility, and improve quality of life.

You will perform comprehensive evaluations, develop individualized treatment plans, provide interventions such as gait and balance training, document progress, and collaborate with nursing, OT, speech, and social services to support

Qualifications

• Active state Physical Therapist license.
• Excellent communication, documentation, and clinical reasoning skills.
• Experience in geriatrics or skilled nursing strongly preferred, but new graduates with a passion for older adults are welcome.

Responsibilities

• Perform comprehensive evaluations for residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions.
• Develop individualized treatment plans tailored to resident goals, functional needs, and SNF regulatory guidelines.
• Provide therapeutic interventions including strengthening, balance training, gait training, transfer training, pain management, and functional mobility retraining.
• Collaborate closely with nursing, occupational therapy, speech therapy, and social services to support interdisciplinary care.
• Document evaluations, daily notes, progress reports, and discharge summaries in accordance with Medicare, state, and facility requirements.
• Educate residents and caregivers on safe mobility, fall prevention, and home exercise programs.
• Maintain a safe and supportive therapy environment.
