Job summary

Outcome HC is seeking a licensed Physical Therapist Assistant to support licensed Physical Therapists in a skilled nursing facility. This full-time role focuses on improving residents' mobility, strength, and independence through therapeutic interventions and collaboration with the care team.

The PTA will document progress, assist with resident assessments, and help implement treatment plans, including gait training and modalities.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current state PTA license in good standing.

Responsibilities

Physical Therapist Assistant (PTA) - Full Time| Skilled Nursing Facility

Location: Mahoning Post Acute Nursing and Rehab

Job Type: Full Time

The Physical Therapy Assistant supports licensed Physical Therapists in delivering high-quality rehabilitation services to residents in a skilled nursing facility. This role focuses on helping residents improve mobility, strength, functional ability, and overall quality of life through therapeutic interventions. PTAs carry out treatment plans, document progress, and collaborate with the interdisciplinary care team to ensure safe, effective, and compassionate care.

Clinical Care

• Provide physical therapy treatments and therapeutic exercises as directed by the supervising Physical Therapist.

• Assist with resident assessments by measuring range of motion, strength, and functional mobility when appropriate.

• Implement treatment plans that may include gait training, balance activities, strengthening, and use of modalities.

• Observe and report changes in resident condition to the Physical Therapist and nursing staff.

Documentation & Compliance

• Accurately document treatments, resident responses, and progress in accordance with facility policies and regulatory requirements.

Resident Interaction

• Provide care with patience, courtesy, and respect, even in challenging situations.

• Educate residents and families on exercises, safety, and functional strategies to support independence.

Collaboration

• Work closely with Physical Therapists, Occupational Therapists, Speech Therapists, nursing staff, and other team members to coordinate care.

• Participate in care plan meetings and contribute to goal setting when appropriate.

Qualifications
