Job summary

Rock River Nursing & Rehab in Fort Atkinson, WI, seeks a Physical Therapist (PT) to lead rehabilitation and help patients regain mobility, strength, and independence.

You will assess, design, and implement individualized treatment plans, work with an interdisciplinary team, and document care to ensure comprehensive treatment. A PT license and at least 1 year of experience are required.

Qualifications

• Bachelor's or Master's in Physical Therapy required.
• Current licensure as a Physical Therapist (PT) required.
• Minimum of 1 year of experience.
• Knowledge of current best practices in physical therapy and patient care.

Responsibilities

• Conduct patient evaluations and assessments to identify physical therapy goals and objectives.
• Develop and implement individualized treatment plans to achieve patient goals.
• Provide direct patient care, including exercises, stretches, and other interventions.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate with patients, families, and healthcare teams to ensure continuity of care.
• Participate in interdisciplinary team meetings to discuss patient care and progress.
• Maintain accurate and detailed records of patient care and treatment plans.

Education

Bachelor's or Master's degree in Physical Therapy
