Job summary

You will assess, design, and implement individualized treatment plans, work with an interdisciplinary team, and document care to ensure comprehensive treatment. A PT license and at least 1 year of experience are required.

Qualifications

• Bachelor's or Master's in Physical Therapy required.
• Current licensure as a Physical Therapist (PT) required.
• Minimum of 1 year of experience.
• Knowledge of current best practices in physical therapy and patient care.

Responsibilities

• Conduct patient evaluations and assessments to identify physical therapy goals and objectives.

About the Role:

As a Physical Therapist (PT) at Rock River Nursing & Rehab, you will be responsible for providing high-quality, patient-centered care to individuals with a variety of conditions, including chronic illnesses, injuries, and post-operative rehabilitation. You will design and implement individualized treatment plans, conduct evaluations and assessments, and collaborate with our interdisciplinary team to ensure comprehensive care.
