Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin is seeking a Physical Therapist (PT) to help patients recover mobility and independence. You will assess, design and implement individualized treatment plans and collaborate with an interdisciplinary team.

Qualifications

Bachelor's or Master's degree in Physical Therapy.
Current licensure as a Physical Therapist (PT).

Responsibilities

Communicate with patients, families, and healthcare teams to ensure continuity of care.
