Job summary

Concierge Home Care seeks a contract Physical Therapist (PT) to provide skilled home health therapy on a 1099 basis across a flexible territory. You will evaluate, treat, and design care plans in patients’ homes, ensuring safe mobility and independence post-illness or surgery.

The role emphasizes OASIS documentation, collaboration with clinicians, and autonomy over scheduling, with compensation negotiable by visit type and complexity. AHCA screening may apply.

Qualifications

• Active Florida PT license required.
• OASIS documentation experience required.
• CPR certification.
• Valid driver’s license with reliable transportation.
• Professional liability insurance optional or required by agency.

Responsibilities

• Perform in-home physical therapy evaluations and treatments.
• Develop and implement individualized care plans.
• Complete Medicare OASIS documentation (SOC, ROC, Recertifications, Discharges).
• Conduct patient evaluations.
• Educate patients and caregivers on mobility, safety, and recovery.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise PTAs when applicable.

Your Role as a Contract Home Health Physical Therapist

As a Contract Physical Therapist, you will provide skilled therapy services in patients’ homes to help them regain strength, mobility, and independence following illness, injury, or surgery.

Responsibilities include:

• Perform in-home physical therapy evaluations and treatments
• Develop and implement individualized care plans
• Complete Medicare OASIS documentation, including:Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges
• Evaluations
• Educate patients and caregivers on mobility, safety, and recovery
• Collaborate with physicians and interdisciplinary care teams
• Supervise Physical Therapy Assistants (PTAs) when applicable

Qualifications

• Active, unencumbered Florida Physical Therapist license
• OASIS documentation experience required
• CPR certification
Valid driver’s license, reliable transportation, and auto insurance
• Professional liability insurance
• Workers’ Comp Exemption or occupational Accident coverage
