Job summary

Concierge Home Care seeks a Contract Physical Therapist (PT) to deliver skilled therapy in patients' homes across a flexible territory. The role emphasizes autonomy, scheduling freedom, and advanced EMR tools to streamline documentation.

Responsibilities include in-home evaluations, care plan development, and Medicare OASIS documentation (SOC, ROC, Recertifications, Discharges). Florida licensure, CPR, and reliable transportation are required.

Qualifications

• Active Florida Physical Therapist license is required.
• OASIS documentation experience is typical in home health.
• CPR certification is expected.

Responsibilities

• Perform in-home physical therapy evaluations and treatments.
• Develop and implement individualized care plans.
• Complete Medicare OASIS documentation for SOC/ROC/Recertifications/Discharges.
• Educate patients and caregivers on mobility and safety.
• Collaborate with physicians and interdisciplinary teams.
• Supervise PTAs when applicable.

Your Role as a Contract Home Health Physical Therapist

As a Contract Physical Therapist, you will provide skilled therapy services in patients’ homes to help them regain strength, mobility, and independence following illness, injury, or surgery.

Responsibilities include:

• Perform in-home physical therapy evaluations and treatments
• Develop and implement individualized care plans
• Complete Medicare OASIS documentation, including:Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges
• Evaluations
• Educate patients and caregivers on mobility, safety, and recovery
• Collaborate with physicians and interdisciplinary care teams
• Supervise Physical Therapy Assistants (PTAs) when applicable

Qualifications

• Active, unencumbered Florida Physical Therapist license
• OASIS documentation experience required
• CPR certification
Valid driver’s license, reliable transportation, and auto insurance
• Professional liability insurance
• Workers’ Comp Exemption or occupational Accident coverage
