Laurel Lakes Rehabilitation and Nursing Center in Chambersburg, PA is seeking a qualified Physical Therapist to provide one-on-one and group therapy to residents under care plans.

You will assess residents, develop and adjust therapy plans with the MDS team, coordinate with other departments, document progress, and support discharge planning while maintaining equipment and safety standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• NEW GRADS ENCOURAGED TO APPLY.
• Must have a valid professional license for state.

Responsibilities

• Providing individual therapy services to residents according to care plans.
• Assisting in resident assessment activities as needed.
• Assisting in developing therapy plans for residents with the MDS.
• Adjusting therapy plans to match resident care goals.
• Briefing residents on procedures and preparing equipment.
• Participating in discharge planning and care plan development.
• Interviewing residents and families as necessary.
• Coordinating therapy appointments with other departments.
• Following therapy department safety standards.
• Maintaining supplies, equipment, and therapy area appearance.

Responsibilities of the Physical Therapist (PT):

• Providing individual therapy services to residents according the care plans.

• Keeping the Therapist informed of any need to adjust therapy plans to match or support resident care plan goals.
• Briefing residents in procedures involved in physical therapy and preparing necessary equipment.
• Participating in discharge planning, development and implementation of resident care plans, resident assessments, etc.
• Interviewing residents and family members as necessary.
• Coordinating therapy appointments with other departments.
• Following therapy department safety standards.
• Maintaining the care and use of supplies, equipment and appearance of the therapy area.

Requirements of the Physical Therapist (PT):

• Graduate of an accredited Physical Therapy program
• NEW GRADS ENCOURAGED TO APPLY
• Must have a valid professional license by discipline for state
• Prefer at least one-year experience in LTC
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Passionate about post-acute rehabilitation and the older adult population
• Ability to relate and interact positively, effectively and appropriately with residents, family, professional and facility staff.
• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
