Job summary

Green Acres Nursing and Rehab in La Plata, MD is seeking a dedicated Physical Therapist to join our rehabilitation team. The PT will assess residents, develop and implement individualized treatment programs, and collaborate with interdisciplinary teams to improve functional outcomes.

We offer a competitive hourly wage of $45–$54 with sign-on and referral bonuses, plus comprehensive benefits and flexible full-time shifts.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid MD state license and registration.
• Experience in long-term care preferred.

Responsibilities

• Assess residents requiring rehabilitation services.
• Provide direct patient care and evaluate medical condition and functional status.
• Establish and manage a treatment program with goals.
• Set achievable goals for patients.
• Document patient condition and educate families.
• Perform evaluations, progress notes, recertifications and discharge paperwork.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform other duties as assigned by Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.

Your Role as a Physical Therapist PT:

At Green Acres Nursing and Rehab, our Physical Therapist PT provides essential care to our residents by:

• Responsible for the assessment of referred patients that require rehabilitation services.

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.

• Evaluations, weekly progress notes, recertification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.

What You Need:

• Graduated from an accredited Physical Therapy college or university

• Valid license and registration for the state of MD

• Experience with long-term care preferred
