Advanced Rehab at Autumn Lake in Lutherville-Timonium, MD is hiring a Physical Therapist (PT) for a full-time role. You will provide individualized therapy to residents, focusing on strength, mobility, balance, and independence.

The position offers an hourly wage of $44-$50, comprehensive benefits including health insurance, 401(k), PTO, CE support, and opportunities for professional development; prior SNF or post-acute rehab experience is preferred but new graduates are welcome to apply.

Qualifications

• Active Maryland Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• New graduate Physical Therapists are welcome to apply.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled treatment addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and track resident progress throughout therapy.
• Modify treatment plans based on clinical needs and response to treatment.
• Educate residents and caregivers on exercises and safe mobility strategies.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs, nursing staff, and other rehabilitation professionals.
• Collaborate with the rehab team to optimize resident outcomes.
