Job summary

Elara Caring seeks a licensed Physical Therapist to provide in-home PT services across the Mishawaka area. You will evaluate patients, develop individualized plans, and monitor progress while collaborating with physicians and the care team.

The role requires graduation from an APTA-approved program, active PT license, and at least one year in a healthcare setting. A valid driver’s license or reliable transportation is needed to travel between patient homes.

As a Physical Therapist, you’ll contribute to our success in the following ways:

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program Current, unrestricted PT license valid for the state of work Minimum one (1) year of experience in a healthcare facility Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
