Job summary

Elara Caring is seeking a Physical Therapist to deliver high-quality care in patient homes in Indiana. You will evaluate patients, develop treatment plans, and provide therapy in a home setting, collaborating with the care team to achieve goals.

Requirements include a PTA-approved program, active PT license, 1 year of healthcare facility experience, and reliable transportation. A sign-on bonus of $15,000 is offered and a fleet vehicle option may be available.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

As a Physical Therapist , you’ll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans.

• Provide physical therapy services in the home setting.

• Collaborate with physicians, caregivers, and the interdisciplinary care team.

• Complete timely and accurate clinical documentation.

• Supervise therapy assistants and support staff, as appropriate.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program

• Current, unrestricted PT license valid for the state of work

• Minimum one (1) year of experience in a healthcare facility

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
