Job summary

Elara Caring in Indiana is seeking a Physical Therapist to deliver in-home care, evaluate patients, and develop individualized treatment plans.

You will provide services in patients' homes, monitor progress, adjust plans, collaborate with physicians, and supervise therapy assistants as needed. A valid PT license and one year of healthcare facility experience are required, along with a valid driver's license or reliable transportation.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift up to 50 pounds.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program

• Current, unrestricted PT license valid for the state of work

• Minimum one (1) year of experience in a healthcare facility

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
