Job summary

Elara Caring is seeking a Physical Therapist to join our team and deliver high-quality home-based care. You will evaluate patients, create individualized treatment plans, and provide therapy in patients’ homes with a focus on safety and outcomes.

You will collaborate with physicians and the care team, document thoroughly, and supervise assistants as appropriate. A valid PT license and driver’s license are required, with travel between patient locations a daily part of the role.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes and work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.

• Monitor patient progress and adjust care plans as needed.

What is Required?

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
