InHome Therapy is seeking licensed physical therapists to provide in-home care to adults and geriatric patients in the Lake County area. You’ll manage a flexible schedule with a consistent caseload and receive support from a therapy-first team, including mentorship and administrative resources.

Compensation includes a per-visit rate plus the IHT Milestone Reward Program, offering up to $15,000 in milestone payments.

Qualifications

• Active PT license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.
• Familiarity with email/EMR and basic technology; training provided.

Responsabilités

• Deliver in-home physical therapy services to adult and geriatric patients in your community.
• Evaluate patient mobility, strength, and function to develop individualized treatment goals.
• Implement effective rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate and timely home health care documentation per Medicare and agency standards.

InHome Therapy is hiring full-time and part-time licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes in and around the Lake County area. Compensation and Benefits*

• The evaluation visit rate for this role ranges from $65-85 per visit

• Basic comfort with technology - email, texting, EMR systems (training provided)

• This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO. Additional offerings may include 401(k), life and disability coverage, voluntary supplemental insurance options, and a company-paid Employee Assistance Program (EAP).
