InHome Therapy is hiring licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes around Hoffman Estates. If you want a flexible schedule, a consistent caseload, and a supportive team, we would love to hear from you.

Responsibilities include delivering in-home PT, evaluating mobility, designing tailored rehab plans, educating patients and families, and maintaining Medicare documentation.

Qualifications

• Active PT license in good standing is required.
• BLS CPR certification required.
• Reliable transportation to see patients in homes.
• Basic comfort with technology (email, texting, EMR systems) with training provided.

• Evaluate patient mobility, strength, and function; develop individualized goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.

• The evaluation visit rate for this role ranges from $65 - $85 per visit

Key Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community
• Evaluate patient mobility, strength, and function to develop individualized treatment goals
• Implement effective rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation per Medicare and agency standards

Requirements

• Active physical therapy (PT) license in good standing

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
