Job summary

Synchrony is seeking a Physical Therapist to provide comprehensive PT services in an interdisciplinary setting. The role includes assessment, treatment planning and therapeutic interventions with focus on patient function and safe practices.

Must hold a current PT license in the state, and CPR certification is preferred. 0–1 year of experience is expected; base responsibilities include patient education, documentation, and collaboration with the care team.

Qualifications

• Current PT licensure in the state of practice.
• CPR certification preferred.
• 0–1 year of PT experience (entry level).

Responsibilities

• Conduct thorough patient assessments and determine rehab needs.
• Develop and implement individualized treatment plans with exercises and modalities.
• Educate patients and families on exercise, posture and body mechanics.
• Maintain accurate documentation of progress and plans per policies.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Perform other duties as assigned.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
