Job summary

French Prairie Nursing & Rehabilitation Center in Woodburn, OR, is seeking a Physical Therapist (PT) to evaluate residents, plan individualized care, and guide therapeutic exercises. You will collaborate with interdisciplinary teams, document accurately, and support safe equipment use while focusing on patient-centered care and measurable progress for post-acute needs.

• Licensed in Oregon or eligible for licensure.
• Graduate of a CAPTE Accredited Physical Therapy Education Program.
• Experience in post-acute care preferred.
• Strong communication and problem-solving skills.
• Commitment to collaboration and patient-centered care.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, functional training, and pain management.

What You’ll Do

What We’re Looking For

• Licensed in Oregon or a graduate of an approved Physical Therapist program or actively working toward licensure




• Graduate of a CAPTE Accredited Physical Therapy Education Program




• Experience in post-acute care or similar healthcare environment preferred




• Strong communication and problem-solving skills




• Commitment to collaboration and patient-centered care
