Ultimate Therapy at Autumn Lake in Vineland, NJ seeks a dedicated Physical Therapist (PT) for a full-time role in its rehabilitation team. You will perform evaluations, create individualized plans, and deliver therapy focused on mobility, strength, and safety for residents.

Qualifications

• Active New Jersey Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and interpersonal skills.
• New graduate PTs welcome; post-acute rehab experience preferred but not required.

Responsibilities

• Perform PT evaluations and develop individualized care plans.
• Deliver skilled treatment addressing strength, balance, gait, transfers, endurance, and mobility.
• Set rehab goals and monitor resident progress.
• Adapt treatment as residents’ needs change.
• Educate residents and caregivers on exercises and safe movement.
• Maintain accurate clinical documentation.

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Vineland. This full-time opportunity is ideal for a therapist who enjoys working with residents to restore movement, build strength, and improve overall function through personalized rehabilitation.

What You’ll Do

• Complete physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment addressing strength, balance, gait, transfers, endurance, and functional mobility
• Establish appropriate rehabilitation goals and track resident progress
• Adapt treatment approaches based on each resident's needs and response to therapy
• Educate residents and caregivers on exercises, mobility strategies, and safe movement techniques
• Maintain accurate and timely clinical documentation
• Coordinate care with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team
