Job summary

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Southgate in Carneys Point, NJ. This full-time opportunity emphasizes helping residents regain mobility and independence through individualized rehabilitation plans.

You will evaluate residents, develop plans of care, provide skilled therapy for strength, balance, gait, and functional mobility, and collaborate with a multidisciplinary team.

Qualifications

• Active New Jersey Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and teamwork skills.
• New graduate Physical Therapists are welcome to apply.

Responsibilities

• Evaluate residents and develop personalized physical therapy plans of care.
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility.
• Establish appropriate rehabilitation goals and track resident progress.
• Adjust treatment plans based on clinical findings and individual needs.
• Educate residents and caregivers on exercises, mobility strategies, and safe movement.
• Complete accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team.

What You'll Do
