Salary based on full-time employment and max productivity: $100,000 to $115,000 per year.

Responsibilities

The Home Health Physical Therapist (PT, DPT, RPT) assesses and evaluates patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates progress.

• Evaluate the patient's functional status and physical therapy needs. Assist the physician in developing the therapy plan of care.
• Observe and report the patient's response to treatment and any condition changes to the supervising nurse or physician.
• Treat patients to relieve pain, develop and restore function, and maintain maximum functioning according to standards of practice.
• Instruct and inform the healthcare team, patient, and family/caregivers about the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Must have a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
