Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

  • Evaluate the patient's functional status and needs; assist in developing the therapy plan of care with the physician.
  • Observe and report the patient's response to treatment and any condition changes to the supervising nurse or physician.
  • Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice.
  • Educate and inform the healthcare team, patients, and families about the plan of care, limitations, and progress.
Education and Experience

Licensure Requirements:

  • Current Physical Therapy license in MA
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle
