Job summary

InHome Therapy is seeking a licensed home health Physical Therapist to serve adult and geriatric patients in and around Skokie. You will deliver in-home PT services with autonomy, evaluate mobility and function, and implement rehab programs to improve patient outcomes.

We offer a flexible schedule, stable caseload through our partner network, and mentoring along with a tablet provided for documentation and communication.

Qualifications

  • Active physical therapy (PT) license in good standing.
  • BLS CPR certification required.
  • Reliable transportation to see patients in their homes.
  • Basic comfort with technology - email, texting, EMR systems (training provided)

Responsibilities

  • Deliver in-home physical therapy services to adult and geriatric patients in your community.
  • Evaluate patient mobility, strength, and function to develop individualized treatment goals.
  • Implement effective rehabilitation and therapeutic exercise programs.
  • Educate patients and families on recovery plans and preventive care.
  • Maintain accurate and timely home health care documentation per Medicare and agency standards

Compensation and Benefits*

  • The evaluation visit rate for this role ranges from $65-$ 85 per visit

*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO. Additional offerings may include 401(k), life and disability coverage, voluntary supplemental insurance options, and a company-paid Employee Assistance Program (EAP).
