Position Summary:

Evaluates, plans, organizes, develops, and directs the Physical Therapy (PT) services for assigned patients/residents in accordance with current applicable laws and regulations, and as may be directed by the Director of Rehabilitation and/or Administrator, to assure that the highest degree of quality care can be maintained.

• Meets with administration, medical and nursing staff, as well as other related departments in planning therapy services
• Maintains treatment records, resident files, and progress notes as required
• Assesses patients’ needs; identifies and initiates appropriate physical therapy interventions
• Observes and evaluates treatment outcomes recommending changes as neededPerforms assessments and re-assessments as needed on each assigned patient/resident
