Victorian Post Acute is seeking a compassionate Physical Therapist to join our in-house rehabilitation team in California. We offer flexible scheduling options (Full-Time, Part Time, PRN) and a sign-on bonus, with pay starting at $60–$65 per hour depending on experience.

Qualifications

• Current Physical Therapist license in good standing.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services per physician orders and care plans.
• Supervise Physical Therapist Assistants and ensure compliance with state practice regulations.
• Collaborate with interdisciplinary team members to coordinate and optimize patient care.
• Maintain accurate documentation including evaluations, daily treatment notes, progress reports, and discharge summaries.
• Educate patients, families, and caregivers on treatment goals, home exercise programs, and safe mobility techniques.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established care plans.
• Supervise Physical Therapist Assistants and ensure compliance with state practice regulations.
• Collaborate with interdisciplinary team members to coordinate and optimize patient care.
• Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.
• Maintain timely and accurate documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.
• Educate patients, families, and caregivers on treatment goals, home exercise programs, and safe mobility techniques.
• Assist with discharge planning and recommendations for continued care.
• Recommend and facilitate acquisition of durable medical equipment as appropriate.
• Monitor and report equipment maintenance needs to ensure a safe treatment environment.
