Job summary

Volare Health LLC seeks a licensed Physical Therapist for Grants Pass, OR, at Umpqua Valley Nursing & Rehabilitation Center. Full-time and PRN shifts available with an in-house therapy team focused on patient care.

You will evaluate patients, design treatment plans, provide therapeutic exercises, educate residents and families, collaborate with care teams, and ensure timely documentation. Competitive benefits and flexible scheduling available.

Qualifications

• Licensed in Oregon or graduate of an approved Physical Therapist program or actively pursuing licensure.
• Graduate of a CAPTE Accredited Physical Therapy Education Program.
• Experience in post-acute care or similar healthcare environment preferred.
• Strong communication and problem-solving skills.
• Commitment to collaboration and patient-centered care.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans

What You’ll Do

What We’re Looking For
